No person, organization, institution or authority has forced, pressured or compelled me to become a member. I certify that all information furnished by me is true and correct to the best of my knowledge. If any information is found false, my membership may be cancelled by the organization. ମୁଁ ଏହା ଦ୍ୱାରା ସ୍ୱଘୋଷଣା କରୁଛି ଯେ, ମୁଁ ମୋର ନିଜ ଇଚ୍ଛାରେ ଉତ୍କଳ ଭାରତ ନିର୍ମାଣ ଶ୍ରମିକ ସଂଘ (UBNSS) ର ସଦସ୍ୟତା ପାଇଁ ଆବେଦନ କରୁଛି। ଏଥିପାଇଁ ମୋତେ କୌଣସି ବ୍ୟକ୍ତି, ସଂସ୍ଥା କିମ୍ବା କର୍ତ୍ତୃପକ୍ଷ ବାଧ୍ୟ କରିନାହାନ୍ତି। ମୁଁ ପ୍ରଦାନ କରିଥିବା ସମସ୍ତ ସୂଚନା ମୋର ଜ୍ଞାନ ଓ ବିଶ୍ୱାସ ଅନୁଯାୟୀ ସତ୍ୟ ଓ ସଠିକ୍।
